A printable invoice will display at the conclusion of the accepted application process - please enclose a copy of this invoice with your check. Membership is held by the individual, not the company, and will stay with the member if company is changed during calendar year.

Associate Member $150 Annually

Associate Memberships are available to persons engaged in or employed by firms providing ANCILLARY SERVICES to the Arizona workers' compensation claims industry, including but not limited to attorneys, nurse case managers, physicians, rehab service providers, investigative service providers, etc.

Regular Member $100 Annually

Regular membership are available to professionals DIRECTLY engaged in the management and / or administration of workers' compensation claims.
